NHS Physiotherapist Salary 2026/27: Rotational to Consultant
Physiotherapists follow a clear band ladder in the NHS. Here is what each stage pays and how long people typically spend at each.
Updated · 4 min read
Physiotherapy has one of the clearest career ladders in the NHS. Most physiotherapists start on rotations through different clinical areas, then specialise, then choose between clinical leadership, advanced practice or management. The same structure broadly applies to occupational therapists, speech and language therapists and dietitians.
Band 5: rotational physiotherapist
Newly qualified physiotherapists registered with the HCPC usually start on Band 5 rotational posts. You typically spend four to six months in each of several areas, such as respiratory care, musculoskeletal outpatients, neurology and orthopaedics.
| Band 5 step | Salary | Monthly take-home |
|---|---|---|
| Entry | £32,073 | £2,040 |
| After 2 years | £34,592 | £2,177 |
| After 4 years | £39,043 | £2,381 |
Take-home pay is for full-time hours with the NHS Pension and tax code 1257L, with no student loan. Most graduates do repay a loan, which reduces take-home by £20 to £70 a month on Plan 2 at Band 5.
Band 5 with a student loan
| Band 5 step | Plan 2 repayment | Take-home with Plan 2 |
|---|---|---|
| Entry | £20 | £2,020 |
| After 2 years | £39 | £2,138 |
| After 4 years | £72 | £2,308 |
If you took a master's route, you may also repay a postgraduate loan at 6% of earnings above £21,000, on top of your main loan. At Band 5 entry that is about £55 a month. The student loan calculator handles both together.
Band 6: specialist physiotherapist
After 18 months to three years, most physiotherapists move to a Band 6 specialist post in one clinical area, or a senior rotational role.
| Band 6 step | Salary | Monthly take-home |
|---|---|---|
| Entry | £39,959 | £2,430 |
| After 2 years | £42,170 | £2,548 |
| After 5 years | £48,117 | £2,866 |
Band 7: highly specialist or team lead
Band 7 posts include highly specialist clinicians, team leaders and first contact practitioners in primary care. Pay runs from £49,387 to £56,515. Many first contact physiotherapists in GP practices are employed on Band 7 or 8a, depending on the employer.
Band 8a and above: advanced and consultant roles
- Advanced practitioner physiotherapists, such as extended scope practitioners in orthopaedics, are often on Band 8a (£57,528 to £64,750).
- Consultant physiotherapists are usually on Band 8b or 8c.
- Therapy managers and heads of service range from Band 8a to 8c depending on the size of the service.
How long does it take to progress?
A typical path looks like this, though it varies a lot:
- Years 1 to 2: Band 5 rotations
- Years 3 to 6: Band 6 specialist
- Years 7 onwards: Band 7 for many, with some moving to 8a later
Each move resets you to the entry point of the new band. The pay progression calculator shows when each future step on your current band is due.
Weekend and on-call work
Many physiotherapists work weekend rotas and respiratory on-call, which add to pay:
- weekend shifts on a Saturday are paid at basic plus 30%, and Sundays at plus 60%, for Bands 5 to 7
- respiratory on-call has local payment arrangements, plus pay for time actually worked when called in
Tax relief on professional fees
Your HCPC registration fee is on HMRC's list of approved professional fees, and so is membership of the Chartered Society of Physiotherapy. You can claim tax relief on both through your personal tax account. Approved claims usually go into your tax code, so you pay a little less tax each month. If you have never claimed, you can usually backdate claims for up to four previous tax years.
Private practice compared with the NHS
Private physiotherapy can pay more per hour, especially for experienced clinicians. But NHS posts include the NHS Pension employer contribution, which is worth around a quarter of salary on top of your pay, plus generous sick pay, maternity pay and annual leave of up to 33 days plus bank holidays. A fair comparison needs to include all of these.
Student physiotherapists
Physiotherapy students in England can usually claim the NHS Learning Support Fund of at least £5,000 a year, with help towards placement travel and childcare.
Common questions
Is a master's degree route paid differently? No. Pre-registration master's graduates also start on Band 5.
Do physiotherapists get London weighting? Yes, at 15% or 20% of basic pay within set limits, depending on the zone.
Can I go straight to Band 6? Not usually. Band 6 posts normally need post-registration experience.
