NHS Band 8b, 8c, 8d and 9 Salaries 2026/27: Senior Pay Explained
From senior managers to directors, here is what the top Agenda for Change bands pay, and how tax and pension rules change at this level.
Updated · 4 min read
The top four Agenda for Change bands cover senior clinical leaders, heads of service, deputy directors and many consultant-level roles outside medicine. At this level, the salary figure on the job advert tells you less than ever about what you will actually take home, because higher rate tax, a 12.5% pension contribution and, at the very top, the loss of the personal allowance all come into play.
Pay scales for 2026/27
| Band | Entry | After 2 years | After 5 years |

| Band 8b | £66,582 | £70,896 | £77,368 |
| Band 8c | £79,504 | £84,346 | £91,609 |
| Band 8d | £94,356 | £100,140 | £108,814 |
| Band 9 | £112,782 | £119,583 | £129,783 |
Monthly take-home at each point
Full time, with the NHS Pension and tax code 1257L, no student loan:
| Band and step | Gross per month | Pension | Tax | NI | Take-home |
|---|---|---|---|---|---|
| 8b entry | £5,549 | £594 | £935 | £279 | £3,742 |
| 8b top | £6,447 | £806 | £1,209 | £296 | £4,136 |
| 8c entry | £6,625 | £828 | £1,272 | £300 | £4,226 |
| 8c top | £7,634 | £954 | £1,625 | £320 | £4,735 |
| 8d entry | £7,863 | £983 | £1,705 | £325 | £4,851 |
| 8d top | £9,068 | £1,133 | £2,126 | £349 | £5,459 |
| 9 entry | £9,399 | £1,175 | £2,242 | £356 | £5,626 |
| 9 top | £10,815 | £1,352 | £2,964 | £384 | £6,115 |
A Plan 2 student loan reduces these figures by between about £280 and £750 a month, depending on salary.
Pension contributions at senior bands
From pensionable pay of £67,668, the employee contribution is 12.5%, the top tier. It applies from early in Band 8b and to everyone above. Contributions are taken before tax, so for someone paying 40% tax, each £1 of contribution costs around 60p. In the top part of Band 9, where income in the personal allowance taper is effectively taxed at 60%, the relief is worth even more.
The £100,000 trap
Once your adjusted net income passes £100,000, you lose £1 of your £12,570 personal allowance for every £2 above that line. Between £100,000 and £125,140, the effective tax rate on that slice of income is 60%.
For senior NHS staff, pension contributions reduce adjusted net income. A Band 9 at the top step has a salary of £129,783, but after a 12.5% pension contribution the taxable figure is about £113,560. That still sits inside the taper, which is why the tax figure jumps so sharply at Band 9.
Ways people manage this include salary sacrifice schemes and additional pension contributions, but both have trade-offs. This is the point where independent, regulated financial advice can pay for itself.
The annual allowance
Senior staff are the most likely to meet the pension annual allowance, which limits how much your pension savings can grow each year with tax relief. It is measured by the growth in your NHS pension, not just your contributions, and a large promotion can push growth above the limit in a single year. Unused allowance from the previous three years can usually be carried forward.
NHS Pensions sends a pension savings statement if your growth exceeds the standard allowance. If you are approaching it, Scheme Pays may let the scheme settle a charge on your behalf in exchange for a reduced pension later.
Typical roles
- Band 8b: senior matrons, heads of therapy services, consultant AHPs in many trusts, lead pharmacists
- Band 8c: heads of nursing for a division, associate directors, consultant clinical scientists
- Band 8d: deputy directors, heads of profession for large services
- Band 9: directors of large services and some executive-level roles outside the board
Board-level directors are normally paid outside Agenda for Change under separate arrangements.
Overtime, on-call and extra work
Bands 8b to 9 are not paid overtime at enhanced rates. Senior staff often take part in management on-call rotas, which are paid under local arrangements. Additional clinical sessions are sometimes paid at plain time.
Questions people ask
Do senior bands get the same annual pay award? Usually yes. In some years, awards for the highest bands have been structured differently, so check the pay circular for the year.
Is London weighting capped? Yes. The high cost area supplement has a maximum in each zone, so at senior bands it is a smaller share of pay.
Should I take a lower-band post nearer home? Some people do, especially near retirement. The band comparison tool shows the exact difference in take-home pay.
