Band 7 is where many staff first meet the 40% tax band. Here is what each pay point means in your account.

Band 7 covers ward managers, sisters and charge nurses, clinical nurse specialists, advanced practitioners in training, team leaders in therapy services and many senior scientific roles. It is a big responsibility step, and also the band where tax starts to bite harder.

Where the 40% tax rate comes in
The higher rate threshold is £50,270. Band 7 entry is just below it, and the upper pay points are above it on paper. But because the NHS Pension is taken before tax, your taxable pay is lower than your salary.
At the top of Band 7, for example, pensionable pay of £56,515 less a 10.7% contribution leaves about £50,470 of taxable pay. Only around £200 a year is taxed at 40%. Add some unsocial hours or overtime and more of your pay moves into the higher band, where each extra pound keeps about 58p after tax and National Insurance, before any student loan.
This is why extra hours feel less rewarding at Band 7. It is also why salary sacrifice schemes, such as cycle to work or electric car schemes, can be better value for higher earners. Our guide to NHS salary sacrifice explains how they work.
Do Band 7s get paid overtime?
Yes. Bands 1 to 7 are paid overtime at time and a half, or double time on public holidays. From Band 8a upwards, overtime is not normally paid at enhanced rates. Band 7 staff on rotas also receive unsocial hours enhancements at the same percentages as Bands 4 to 6: 30% for nights and Saturdays, 60% for Sundays and public holidays.
Typical Band 7 roles
- Ward manager, sister or charge nurse
- Clinical nurse specialist
- Trainee advanced clinical practitioner (many trusts pay qualified ACPs at Band 8a)
- Team lead or highly specialist physiotherapist, OT or speech and language therapist
- Senior paramedic or specialist paramedic
- Midwife team leader or specialist midwife
Band 6 to Band 7: what the move is worth
| From | Salary | Band 7 entry | Gain |
|---|---|---|---|
| Band 6 entry | £39,959 | £49,387 | £9,428 |
| Band 6 after 2 years | £42,170 | £49,387 | £7,217 |
| Band 6 after 5 years | £48,117 | £49,387 | £1,270 |
As with Band 5 to 6, the immediate rise is small if you are already at the top of Band 6. The long-term gain comes from the Band 7 steps.
The pension side of Band 7
At the top of Band 7 your contribution rate rises to 10.7%. That can feel like a pay cut, but the pension you build each year also grows in line with your higher pay. In the 2015 scheme you earn 1/54th of your pensionable pay as pension each year. On £56,515, that is about £1,047 of yearly pension added for each year worked, before revaluation. The pension retirement calculator projects what this could be worth.
Quick answers
