Band 3 covers experienced support staff: senior healthcare assistants, clinical support workers who carry out tasks like phlebotomy and ECGs, pharmacy assistants, senior admin staff and many others. It is the first band with a pay step, so your salary rises after two years without you having to change jobs.
Band 3 pay points for 2026/27
| Pay point | Annual salary | Hourly rate |
|---|---|---|
| Entry | £25,760 | £13.17 |
| After 2 years | £27,476 | £14.05 |
The step up after two years is worth £1,716 a year before deductions. Under the pay progression system introduced in 2019, the step is not completely automatic. Your manager should hold a pay step review to confirm you have completed mandatory training and an appraisal, and that no formal capability or disciplinary process is in place. In practice most people move up on time, but it is worth checking your training record a few months before your step date. Our guide to NHS pay progression explains the process.
Monthly take-home on Band 3
For a full-time employee with the NHS Pension and tax code 1257L:
| Entry | After 2 years | |
|---|---|---|
| Gross per month | £2,147 | £2,290 |
| NHS Pension (6.5%) | £140 | £149 |
| Income tax | £192 | £219 |
| National Insurance | £88 | £99 |
| Take-home | £1,727 | £1,823 |
Neither pay point is above the Plan 2 student loan threshold of £29,385, so Plan 2 borrowers will not normally see a deduction on basic pay alone. Plan 5 borrowers (threshold £25,000) will see a small deduction at both pay points, and Plan 1 borrowers (threshold £26,900) at the upper step.
Unsocial hours on Band 3
Band 3 has its own enhancement rates, which sit between Band 2 and the bands above:
- Weekday nights (8pm to 6am) and Saturdays: plus 37% of the basic hourly rate
- Sundays and public holidays: plus 74%
At the entry point, a Sunday hour is worth about £22.92 and a Saturday hour about £18.04. Staff who regularly work weekends can add a significant amount to their annual pay. The unsocial hours calculator shows the effect of your actual rota.
Who is paid on Band 3?
Typical Band 3 roles include:
- clinical support workers and senior healthcare assistants
- phlebotomists
- maternity support workers in some trusts
- pharmacy assistants and dispensers
- medical secretaries and senior administrators in some trusts
- estates and maintenance assistants
The deciding factor is the job description. If your role includes clinical tasks that are described at Band 3 in the national job profiles but you are paid Band 2, a banding review may be appropriate. Speak to your union or HR before raising it formally.
Band 3 compared with nearby bands
| Band | Entry | Top |
|---|---|---|
| Band 2 | £25,272 | £25,272 |
| Band 3 | £25,760 | £27,476 |
| Band 4 | £28,392 | £31,157 |
The gap between Band 3 entry and Band 2 is small, but the top of Band 3 is more than £2,200 above Band 2. Moving to Band 4 adds a further step after three years. You can compare all bands side by side with the band comparison tool.
Routes on from Band 3
Common next steps include:
- Assistant practitioner or nursing associate roles, usually Band 4.
- Registered nurse or allied health degree apprenticeships, which lead to Band 5 on qualification.
- Senior admin and team leader roles at Band 4.
If you are thinking of a degree while working, read about the NHS Learning Support Fund, which is available to many healthcare students in England.
Leave, sick pay and pension
Your leave and sick pay entitlement depend on years of NHS service, not your band. You start on 27 days of annual leave plus 8 public holidays. Your NHS Pension contribution on Band 3 is 6.5%, and the employer pays a much larger share on top.
Common questions
When does my pay step happen? Two years after you started on the band, assuming your pay step review is completed. If you joined on a pay point higher than entry, your step date may be different.
Does previous experience count? Starting pay is normally the entry point. Some employers agree a higher starting point for relevant experience, but you need to agree it before accepting the offer.
Is overtime worth it on Band 3? Overtime is paid at time and a half. Once tax and NI are taken off, around 70% of the extra gross pay reaches your account. The overtime take-home calculator shows the exact figure.
Try your own numbers
