Advanced clinical practitioners (ACPs) are experienced registered professionals, such as nurses, paramedics, pharmacists, physiotherapists and others, who have trained to a master's level in advanced practice. They assess, diagnose, prescribe and manage care independently, often in emergency departments, acute medicine, primary care and specialist services.

What the move from Band 7 to 8a is worth

If you trained as an ACP on Band 7 entry and qualify into Band 8a, your basic pay rises by £8,141 a year and your take-home by about £419 a month. If you were already at the top of Band 7, the immediate rise is only about £1,013 a year, or roughly £44 a month in take-home pay.

The overtime trap at Band 8a

This catches many new ACPs out. Under Agenda for Change, overtime on Band 7 is paid at time and a half. Staff on Band 8a and above are not normally paid overtime at enhanced rates, and extra hours are usually managed as time off in lieu or paid at plain time under local policy.

ACPs in emergency departments often work shifts, and unsocial hours enhancements still apply on Band 8a: plus 30% for weekday nights and Saturdays, and plus 60% for Sundays and public holidays. But if you used to earn a lot of Band 7 overtime, your total income may not rise as much as the basic salaries suggest. Compare your actual annual earnings before and after with the overtime take-home calculator.

Training to be an ACP

The usual route is:

  1. Several years of post-registration experience in your profession, often at Band 6 or 7.
  2. A trainee ACP post, combining clinical training with a master's degree in advanced clinical practice, usually over three years part time.
  3. Independent prescribing, which is part of most ACP programmes for eligible professions.

Training covers the four pillars of advanced practice: clinical practice, leadership and management, education, and research. Employers usually fund the master's degree for trainee posts. Check whether you will need to repay any funding if you leave early.

Tax at ACP salaries

On Band 8a, most of your pay above about £50,270 of taxable income is taxed at 40%, and pension contributions are 10.7%. At the top of Band 8a, roughly £7,500 a year is taxed at the higher rate. Pension contributions are taken before tax, so they cost you less than they appear. The take-home pay calculator shows the exact figures.

Common questions

Do ACPs get paid more than nurses in the same department? Yes, because ACP posts are usually banded higher, but the gap shrinks once overtime and enhancements are included.

Is a physician associate the same as an ACP? No. Physician associates are a separate profession with different training and regulation.

Can paramedics and pharmacists become ACPs? Yes. ACPs come from many registered professions.

Recognition of advanced practice

Because ACP banding is decided locally, recognised standards matter. In England, NHS England's Centre for Advancing Practice accredits advanced practice master's programmes and recognises the experience of practitioners who trained in other ways. Many employers now expect ACPs to have completed an accredited programme or gained recognition this way, and it can help when applying for Band 8a posts in other trusts.

Advanced practice is not a separately regulated title. You remain regulated by your own professional body, such as the NMC, the HCPC or the GPhC, and work within your own scope of practice.
