NHS District Nurse Salary 2026/27: Community Nursing Pay Explained
Community nursing has its own career ladder, built around the district nursing qualification. Here is what each stage pays.
Updated · 4 min read
District nurses care for people in their own homes and in the community, often managing complex conditions, wounds, end-of-life care and medicines for patients who cannot easily get to a clinic. The title "district nurse" is linked to a specialist qualification, and the pay ladder follows it.

The district nursing qualification (SPQ)
To be recorded as a district nurse with the NMC, you complete a Specialist Practitioner Qualification in district nursing. It is usually a one-year full-time or two-year part-time programme at degree or master's level, and it often includes community nurse prescribing.
Many employers sponsor community staff nurses through the SPQ, paying a salary during the course in return for agreeing to stay for a period afterwards. Read any repayment clause carefully before you sign.
Mileage: a big part of community pay
District nurses can drive thousands of miles a year. Under the NHS mileage scheme you can claim 56p a mile for the first 3,500 business miles in the year, and 21p a mile after that.
A nurse who drives 400 business miles a month passes 3,500 miles during the ninth month of the year. Over the year that is 4,800 miles, and the claim comes to about £2,233. HMRC compares your total mileage payments with its approved rate of 45p a mile. Once your NHS payments fall below the approved amount, which happens at around 5,100 miles a year, you can claim tax relief on the difference. The mileage calculator works out claims month by month.
Hours, weekends and evenings
Many district nursing services run seven days a week and into the evening, with some areas also providing overnight cover. Unsocial hours enhancements apply in the usual way for Bands 5 to 7: plus 30% for weekday evenings after 8pm and Saturdays, and plus 60% for Sundays and public holidays.
Is community nursing paid less than hospital nursing?
Basic pay is the same for the same band. Hospital nurses often earn more from unsocial hours, because ward rotas include more nights. Community nurses often gain from mileage claims, more regular hours and, once qualified, earlier access to Band 6 and Band 7 caseload roles. For many people, the regular pattern is worth more than the enhancements.
Common questions
Can I join district nursing as a newly qualified nurse? Yes. Many teams recruit newly qualified nurses on Band 5 with a preceptorship programme.
Do I need my own car? Usually, yes. Some services offer pool or lease cars.
Is lone working safe? Employers must have lone working policies, including check-in systems and training.
Prescribing in community nursing
Prescribing is a big part of district nursing, and it can help with progression. There are three main levels:
- V100: community practitioner nurse prescribing, usually included in the district nursing SPQ. It allows prescribing from a limited formulary of items such as dressings and some medicines.
- V150: the same formulary as V100, taken as a standalone course by nurses who are not doing the SPQ.
- V300: independent and supplementary prescribing, a longer course that allows prescribing across a much wider range, within your competence.
Many Band 7 and advanced community roles ask for V300. Employers usually fund prescribing courses for staff who need them in their role. Check whether funding comes with a commitment to stay for a period afterwards.
