Band 8a brings a higher salary but also new rules on overtime and a 40% tax rate on part of your pay. Here is the full picture.

Band 8a is the first of the senior bands. Matrons, advanced clinical practitioners, consultant-level allied health professionals in some trusts, service managers and principal scientists are typically paid here. It is a meaningful pay rise, but several things change at the same time, and some catch people out.

Three things that change at Band 8a
1. Overtime is not paid at enhanced rates
Under Agenda for Change, staff on Band 8a and above are not normally paid overtime at time and a half. Extra hours are usually managed through time off in lieu, and some employers pay plain time for additional sessions. Check your own trust's policy. This can mean that a Band 7 who works a lot of overtime earns more than a newly appointed Band 8a.
2. Unsocial hours rules may differ
If you work shifts on Band 8a, enhancements still apply, but many 8a posts are office-based with on-call arrangements instead. On-call has its own payment rules, agreed locally under the national framework.
3. More of your pay is taxed at 40%
After pension contributions, most Band 8a pay is above the £50,270 threshold. At the top step, taxable pay is about £57,800, so roughly £7,500 a year is taxed at 40%. National Insurance on earnings above £50,270 is only 2%, which softens the effect slightly.
Is the move from Band 7 to 8a worth it?
| From | Salary | Band 8a entry | Gross gain | Monthly take-home gain |
|---|---|---|---|---|
| Band 7 entry | £49,387 | £57,528 | £8,141 | about £419 |
| Band 7 top | £56,515 | £57,528 | £1,013 | about £44 |
From the top of Band 7, the immediate gain is very small. The case for 8a is the higher ceiling of £64,750 and the career route into 8b and beyond. If you currently earn a lot of Band 7 overtime, compare your real annual income before and after, not just the basic salaries.
Typical Band 8a roles
- Matron (some trusts use 8a, others 8b)
- Advanced clinical practitioner once qualified
- Consultant or lead allied health professional in smaller services
- Principal clinical scientist or pharmacist
- Service or operational manager
- Senior project and programme managers
Pension at Band 8a
Contributions are 10.7% across most of Band 8a. Above pensionable pay of £67,668 the rate becomes 12.5%, which applies from Band 8b. The pension builds at 1/54th of your pay each year, so a Band 8a at the top step adds about £1,199 of yearly pension for each year worked.
Higher earners should also keep an eye on the pension annual allowance, which limits tax-free pension growth each year. Most Band 8a staff will be well inside it, but large pay rises in one year, or long service in the older 1995/2008 scheme, can create a charge. NHS Pensions can provide a pension savings statement.
Common questions
Do I get paid for being on call? Yes, through local on-call arrangements. The rate and rules are set by your employer within national guidance.
Is 8a a management band? Often, but many 8a posts are senior clinical roles, especially advanced practice.
Can I negotiate a higher starting point? Starting above entry is discretionary, but senior posts sometimes have more room, especially for hard-to-fill roles.
