NHS Ambulance Jobs Salary 2026/27: Call Handler, ECA and Technician Pay
You do not need a degree to work for an ambulance service. Here is what call handlers, ECAs and technicians earn, and how shift pay works.
Updated · 4 min read
Ambulance services employ far more than paramedics. Call handlers answer 999 and NHS 111 calls, emergency care assistants (ECAs) crew ambulances alongside paramedics, and ambulance technicians respond to emergencies with a wider range of clinical skills. All are paid on the Agenda for Change scale, and many of these roles do not need a degree.

How unsocial hours work in ambulance services
Ambulance services run around the clock, and how you are paid for unsocial hours depends on your contract.
The ambulance arrangement (often called Annex 5). Many frontline ambulance staff receive unsocial hours as a percentage of basic pay, based on how much of their rota falls in unsocial hours, up to 25%. It is paid every month, including during annual leave, and it counts towards your pension.
For a Band 3 ECA at entry with a 25% payment, total pay is £25,760 plus £6,440, which comes to £32,200 a year.
The standard arrangement. Other staff, including many in control rooms and NHS 111, are paid an enhancement for each unsocial hour worked:
| Band 3 | Band 4 | |
|---|---|---|
| Weekday nights (8pm to 6am) and Saturdays | plus 37% | plus 30% |
| Sundays and public holidays | plus 74% | plus 60% |
A Band 3 call handler working a Sunday earns about £22.92 an hour at entry. The unsocial hours calculator shows the effect of a whole rota.
Your contract will say which arrangement applies to you.
What each role involves
- Call handlers take 999 or 111 calls, using structured systems to assess callers and decide the right response. Training is given on the job.
- Emergency care assistants drive ambulances under emergency conditions and support paramedics with patient care. You usually need a full driving licence with the C1 category, or to gain it.
- Ambulance technicians respond to emergencies, assess patients and give a range of treatments, often working with an ECA or paramedic.
Routes to becoming a paramedic
Many paramedics started as ECAs or technicians. Ambulance services often support staff to train through:
- paramedic degree apprenticeships, paid while you study
- university degrees, eligible for the NHS Learning Support Fund
Our paramedic salary guide explains what paramedics earn.
Overtime and late finishes
Overtime is paid at time and a half on Bands 2 to 7, or double time on public holidays. Late finishes are common on the road, so make sure every overrun is recorded and claimed.
Common questions
Do I need a C1 licence to apply? For ECA and technician roles, usually yes, or you must obtain it within a set time. Some services help with costs.
Are call handlers paid shift enhancements? Yes, under whichever unsocial hours arrangement applies.
Is ambulance pay higher in London? London-based staff receive the high cost area supplement on top of basic pay.
Take-home pay with the 25% unsocial hours payment
For frontline staff on the percentage arrangement, the payment makes a big difference to take-home pay. Full time, with the NHS Pension and tax code 1257L:
| Role and step | Basic salary | With 25% payment | Take-home per month |
|---|---|---|---|
| ECA, Band 3 entry | £25,760 | £32,200 | £2,047 |
| Technician, Band 4 entry | £28,392 | £35,490 | £2,191 |
Because the payment is pensionable, it can also move you into a higher pension contribution tier: 8.3% for the ECA example and 9.8% for the technician. Compare that with £1,727 and £1,874 a month on basic pay alone.
Work out your pay
Use the take-home pay calculator. If you are on the percentage arrangement, add your unsocial hours payment to your salary.
